逐点回复信骨架

复制到 <task_dir>/response/ 按审稿人拆文件:00_cover_letter.md(可选)、01_editor.md(有编辑要求时)、02_reviewer1.md、03_reviewer2.md… 渲染时整目录拼接,按文件名序即为信内顺序。语言按 spec §2(下面给 en 骨架,zh 对应替换)。

• 位置只写真实存在的章节 / 图表号;行号用户没给就用章节名,不编行号
• 数据没到的条目保持可见占位:[AUTHOR_INPUT_NEEDED: 需 R1.2 的检验名称与样本量]
• 反驳条目按 tone_and_stance.md 五步结构写,不用指责语
